JAMMU, Mar 10: Laparoscopic Resection of Intestinal Tumor was performed at Government Hospital Gandhi Nagar by Dr Sandeep Bhat (Laparoscopic and Gastrointestinal Surgeon) and his team.
A sixty three year old patient was suffering from sub acute intestinal obstruction with recurrent vomiting and was not able to eat for about a month or so. The patient was admitted in Gandhi Nagar hospital, evaluated and was found to have a block/tumor in the small intestine on CT scan.
The patient was planned for diagnostic laparoscopy which revealed a tumor in the proximal jejunum/small intestine approximately 15-20 cm from duodenum without any evidence of spread/metastatic disease. Resection of the tumor along with the vascular mesentery was done laparoscopically using mono polar cautery and clipping of the vascular pedicles. Intestinal staplers were used for resection and anastamosis. The specimen was retrieved through a very small lower midline incision (3-4cms only).
Patient behaved well in the post operative period. As the procedure was done laparoscopically, analgesics (pain killers) were stopped after second post-operative day and the patient was ambulatory on day two. Oral sips were started on pod 2, followed by liquid and soft diet on day 3 and 4, respectively. Drains were removed on 4th day and patient was discharged on 5th day in a satisfactory condition.
Mostly such cases are operated by open method as the laparoscopic approach merits profound technical expertise and exquisite surgical skills and experience. Also open approach has its own morbidity and late post operative complications thereby adding upon the stress of the already stressed patient.
Biopsy of the specimen revealed moderately differentiated Adenocarcinoma. Patient was referred to medical oncologist for further management. The surgical team was comprised of Dr Sandeep Bhat, Rajinder and Rakesh Bakhshi (both senior Lapro assistants), Sudesh and Gulzari. The anaesthesia team comprised of Dr Laughy Bhan, Dr Anu Manhas, Dr Madhavi and sister Anita.
