JAMMU, Sept 21: Rotary Club of Udhampur and Rajouri is organizing a free heart check-up camp for the persons below the age of 18 years on September 30 at Super Speciality Hospital, Bakshi Nagar.
The camp is being organized in collaboration with Fortis Escorts Heart Institute, New Delhi and Gift of Life, Delhi NCR. The timing of the camp will be from 9 AM to 4 PM.
During the camp, under privileged patients below the poverty line and under the age of 18 years will be screened at Super Specialty Hospital, Bakshi Nagar, Jammu and those who are selected for surgery will be operated at Fortis Escorts Heart Institute, New Delhi free of cost.
The main objective of the camp is to screen the patients who have acute heart problems under the age of 18 years. Such patients must submit his/her recent echocardiography report by 28th September in the office of Rotary Hospital Udhampur.
For screening, the patients are mandatory to bring with them their latest Echocardiography report, 2 photographs, BPL card/Income Certificate (Income upto Rs 8000 per month) from concerned Tehsildar and Age & Address Proof of patient (Aadhar Card). Without these documents Rotary Hospital and Fortis Escorts Heart Institute will not consider any child for screening.
