(a) Permanent Residence Certificate.

(c) Parents/Guardian Income Certificate from concerned Tehsildar in case of students whose parents/guardian are non-govt. employees. In case of employed Parents/guardians salary certificate of concerned Drawing and Disbursing Officer is required along with affidavit that the income from all sources does not exceed the prescribed limit.

Income slab in case of SC/ST student Rs. 65, 290/- Annual Income slab in case of OBC/PH Students is Rs. 44,500/- annual.

(d) Marks certificate (Attested copies by gazetted officer) of the last three years examination passed.

(e) Bonafied Certificate of the concerned institution.

(f) Passport size photographs to be pasted on application forms.

(g) Free receipts of the concerned course.

(h) Promotion certificate in case the student has passed the previous examination for which scholarship already received.

TENDER NOTICE NO. 2/2004

For and on behalf of the Governor of Jammu and Kashmir State, sealed tenders (in duplicate) affixed with Rs. 5/- revenue stamps are invited for supply of items under "Group-II - a) Ayurvedic/Unani Drugs/Medicines and b) Machinery and equipment" as per the details given in the tender documents. Tender form, terms and conditions of N.I.T. and list of items to be provided against cost for use in various Institutions/Hospitals of the Health and Medical Education Department for the year 2004-05 from the original manufacturers/registered firms/approved suppliers/authorized suppliers or those who are dealing actually in the goods for which tenders are issued, on the terms and conditions which can be had from the following offices:

i. Director Indian Systems of Medicines, J&K, Srinagar/Jammu

ii. Deputy Director, ISM Jammu/Srinagar

The tenders are to be submitted on the prescribed tender form/documents which can be obtained on application against non-refundable cash payment of Rs. 500/- in general and Rs. 250/- for local SSI Unit holders from the above mentioned offices upto 10.11.2004 1 P.M.

The tender must be submitted in properly sealed double envelops duly superscribed as Group-II-A)Ayurvedic/Unani Drugs/Medicines and b) Machinery and equipment addressed to the Chairman, Central Purchase Committee, Health and Medical Education Department and should reach the office where from tender documents have been purchased by or before 16.11.2004 upto 3.00 P.M. The sealed tender should be handed over personally at the concerned office and receipt obtained or can be sent by a registered post. The tender will be opened by the Central Purchase Committee or by an officer duly authorized in this behalf by the Chairman/Member Secretary of the Central Purchase Committee on 19.11.2004 at 3 P.M. or any subsequent and convenient date, after tenders are received, in presence of any intending supplier or any other authorized representative who may be present at the time of opening of tenders in the Directorate of ISM, J&K, Jammu.

The purchase committee is not bound to accept the lowest tender and may reject all tenders or any tender or any part of the tender without assigning any reasons thereof.

The intending tenderer(s) shall have to deposit an amount of Rs. 20,000/- (Rs. Twenty thousand) for Group-II - A) Ayurvedic/Unani Drugs/Medicines and B) Machinery and equipment as earnest money in the shape of CDR/FDR from any Nationalized/Scheduled Bank preferably J&K Bank Ltd having validity of not less than two years from the date of issue, pledged to the Member Secretary, Central Purchase Committee, Health and Medical Education Department. The tender without earnest money shall be out rightly rejected.

The rates should be quoted against each item of the tender documents both in figures and in words without cuttings and tamperings.

The tenders received after prescribed time and date will not be entertained.
