Subject:- Opening of Fair Price Medical Shops/Canteens/STD booths in the Hospitals of Jammu Division

NOTICE

In order to streamline the policy of allotment of Fair price Medical shops/Canteens/STD booths in the Hospitals in Jammu Division and to effect the transparency in the system, the Health and Medical Education Department vide Govt order No. 708-HME of 2003 dated 4.06.2003 to be read with Govt order No. 02-HME of 2004 dated 1.1.2004, it has been decided that the Health Department would propose to open Fair Price Medical Shops/Canteens/STD booths in Hospitals of Jammu Division through open auction duly publicized in the local press as well as electronic Media to provide these facilities in the hospitals round the clock for the patients as well as for the public.

The following should be the terms and conditions prescribed for auctions:

1. The bidder shall have obtained a retail drugs license for sale of medicines issued by the competent authority under Drugs and Cosmetic Act. 1946 and rules made there under.

2. The allotment of the shops shall be made to the successful bidders on license basis initially for a period of three years extendable by a period of another two years, subject to satisfactory performance of the licensee on the basis of highest premium offered in addition to the monthly rent to be fixed and revised by a committee constituted for this purpose.

3. The allottee shall have to take an undertaking from the successful bidder before handing over the possession to the effect that the said shop shal remain open for 24 hours so as to keep available medicines to the patients and shall not close its shop on account of any strike or other disturbance.

4. The successful bidder shall also have to undertake to put on sale only quality controlled medicines of reputed manufacturers and shall not store or sell any substandard/spuriuos drugs and drugs which are expired and shall adhere strictly to the regulations of Drugs and Cosmetic Act 1946.

5. The successful bidder/licensee shall also have to ensure to take necessary steps to store medicines/drugs in accordance with the guide-lines issued by the Drugs Control Organization and sell such medicines on reasonable rates, on the basis of approved list which shall be displayed by him at a prominent place in the shop so allotted. These term and conditions of licensee for allotment of shops shall always be deemed to be part of the notice inviting tenders.

6. The above terms and conditions shall also be applicable for establishment of canteens and STD booths where applicable.

7. Any violation of above mentioned conditions shall make the bidder/licensee liable for action under rules including termination of the license/cancellation of allotment of shop without any notice.

8. The successful bidder/licensee, shall have to execute an agreement deed for the allotment of the shop/establishment with the allottee where under terms and conditions of the license shall be fixed.

Therefore, it is notified for information of all legible persons to participate in the open auction for allotment of Fair Price Medical Shops/Canteens/STD booths in the Hospitals of Jammu Division. NOTICE INVITING TENDERS

For and on behalf of Governor of Jammu and Kashmir sealed tenders are invited affixed with Rs 2/- Revenue Stamp on plain paper from reputed printers of the state for printing and binding etc of ''DEHAT SUDHAR'' an English Urdu six monthly double faced magazine. Three Thousands Copies approximately of each edition comprising upto 120 pages shall have to be printed with photo graphs to be provided by the department. The calligraphy of Urdu portion and computer page making of English portion (Lazer Printed) is to be done by printers themselves. Before sending the calligraphed computer composed script to the press, it is to be got vetted from Editorial Board of the magazine for approval. The placement of contract is subject to fulfillment of following terms and conditions.

i. Multicolor title cover on 230 GMS art card with scanning and planning.

ii. Printing of upto 120 pages multi-colour planned and scanned on imported 110 GMS Art paper.

iii. The number of copies/pages to be printed can increase or decrease.

iv. Central stiching (Special binding)

v. After receiving the script by printers magazine is to be printed within Ten days.

vi. The magazine is to be delivered at the office premises of this department i.e. Civil Secretariat, Srinagar/Jammu.

vii. Sealed tenders must be submitted along with earnest money of Rs 5000/- (Rupees Five Thousand only) in shape of CDR pledged to Financial Advisor/Chief Accounts Officer, Rural Development Department, Civil Secretariat Srinagar.

viii. Tenders shall be addressed to Financial Advisor/CAO Rural Development Department, Civil Secretariat, Srinagar should reach his office on or before 2nd July 2004 upto 4 PM in seal covers. In case last day happens to be a holiday the tenders shall be received on the next day upto 4 PM.

ix. No tender without CDR shall be accepted.

x. Only those printers having their own offset press facilities can offer tenders. Necessary documentary proof in this behalf needs to be enclosed with the offer. Providing of wrong information would make the offer liable for rejection.

xi. Tenders must be accompanied with requisite paper samples both for inner pages and title covers.

xii. Tenders shall be opened by a committee of members on the next woking date at 11 AM or any subsequent date convenient to convener of the committee. The committee shall have the right to accept or to reject any tender without assigning any reason thereof.

xiii. Vague and ambiguous tenders will not be entertained. There should be no errors/overwritings.

xiv. The sealed envelopes shall be superscribed with words Tender for dehat sudhar.
